Juror Qualification Questionnaire

Pursuant to Title 28, United States Code, Section 1865(b), any person that meets the following criteria is deemed qualified to serve on grand and petit juries:

  • is a citizen of the United States
  • is at least 18 years old
  • has resided primarily in Puerto Rico for at least one year
  • is able to read, write, speak and understand the English language
  • has no pending felony charges against them
  • if convicted of a felony charge, has their civil rights restored
  • has no physical or mental disability that would interfere with or prevent service as a juror

Jurors in the District of Puerto Rico are selected at random from the certified lists of registered voters from the State Elections Commission of the Commonwealth of Puerto Rico. This random selection is done every four (4) years after each election, and is used to select both grand and petit jurors.  

Juror Qualification Questionnaires are mailed out to each randomly selected person.  Within 10 days of receipt, prospective jurors must submit this questionnaire online, or complete the paper form, sign it, and return it in the attached business reply envelope. 

Important Note

  • The Juror Qualification Questionnaire is not a summons for jury service, and recipients are not being called to report at this time.
  • There is a space on the back of the form to add additional messages.
  • A doctor's note is required to claim medical hardship.
  • Recipients that recently served on jury duty in another court must include a copy of their jury certificate. 

Once the answered questionnaires are received at the Jury Administration Office, prospective jurors are qualified based on the information provided. Those found to be qualified for jury service will receive a Jury Service Summons at a later date.
